Chain of Custody Forms

Chemtech-Ford uses several types of custody forms for sample submission. It is important to use the proper form as the different regulatory programs require slightly different information to comply.  We have incorporated this changes into the different COCs.

We have provided an example form to help you understand how to fill out the different sections. If you have questions, please contact your Chemtech-Ford Project Manager at 801.262.7299.

Download this PDF

  • Save it where you can find it again.
  • Open the file and add your content.
  • Save as a PDF under a new name.
  • Print and sign.

 

 


DWCOCEditable Drinking Water Excel Version

Download this zip file

  • Save the file where you can find it again.
  • Using Windows Explorer, find the zip file.  Click on the zip file.
  • The Excel COC file will now appear as 'Chemtech_COC.xlsx'.  Save the file as your master excel file.
  • Edit the Excel file with your sample information.  You will probably need to 'Enable Editing' at the top of the screen.

 


Drinking Water Micro COC

  • Save the file where you can find it.
  • Using Windows Explorer, find the zip file 'DW_Micro_COC.xlsx'.  Click on the zip file.
  • The Excel Microbiology COC file will now appear as 'DW_Micro_COC.xlsx'.  Save the file as your master Excel file.
  • Edit the Excel file with your sample information.  You will probably need to 'Enable Editing' at the top of the screen.

 

 


Wastewater COC

  • Save the file where you can find it again.
  • Using Windows Explorer, find the zip file called 'NDW_COC_Template.xlsx'.  Click on the file.
  • The wastewater Excel file will appear.  Save the file as your master Excel wastewater template.
  • Edit the Excel file with your sample information.  You will probably need to 'Enable Editing' at the top of the screen.
